• Bring your completed forms with you to your appointment. If you cannot print them out at home, please show up 15 minutes early for your appointment to fill them out in our office.

HEALTH HISTORY - REQUIRED NEW PATIENT FORM

This gives us  your medical history and an idea of your current state of your health. When you come in for your initial consultation, Dr. Rathbone will clarify any pre-existing conditions and go over any questions, concerns, and/or goals you have!

Parents: please fill out the Pediatric Form if your child is under 8 years old and the Adult Intake Form if your child is 8 or older.
